Questionnaire listing personal data and places of employment, and discussing Levinger's early interest in physics; his undergraduate education at the University of Chicago; technically detailed discussions of his work in nuclear photoeffect calculations, calculations on few-nucleon systems, and the perturbation approach to the many-nucleon problem (with references); his work with Fabr ̌de la Ripelle at the Universit ̌de Paris; and a brief discussion of public attitudes towards science. Recipients were asked to discuss their entry into a scientific career in general and nuclear physics in particular, their work during the Second World War, their scientific achievements, the funding of their projects, the roles of related sciences--especially high energy physics--in their research, their work as educators, their perceptions of the public's attitude towards science, and their involvement in physics-related organizations.
